After contacting ABC KIDZ for a childcare spot, a tour must be completed to receive an enrollment packet.  There is a non-refundable enrollment fee of $50 per family that is required. Once the enrollment packet is completed and returned, the enrollment fee has been paid, and a contract of care has been signed, your child’s spot is reserved.

An enrollment packet consists of the following:

• Emergency Contact Form

• Immunization Record

• IEP/IFSP Form

• Social Competence and Behavior Questionnaire

• Regulated Childcare Form

• All about Your Child Form

• Discipline Policy

• Contract

• Enrollment Fee

Your child will not be able to attend if any required information is missing or incomplete or if you fail to provide updated immunizations.
